Children's Medical Services

Program Purpose
The purpose of the Children’s Medical Services Program is to provide coordinate, and/or pay for specialized medical treatment for children and adolescents with a diagnosis of one or more of the 200 medically chronic conditions.
Eligibility
Eligibility for the program is based on the following:
- Type of medical condition
- Age of child
- Georgia Resident
- Annual Family Income
Services Provided
CMS offers an array of services which includes:
- Long term treatment and therapy
- Comprehensive physical assessment
- Diagnostic testing
- Durable (i.e. wheelchairs and braces) and disposable medical equipment
- Case management is provided by the CMS nursing staff
- Physicians at the CMS outpatient clinics evaluate each child’s condition, design a treatment plan and follow-up care

- Burns – acute care and reconstructive surgery
- Cardiac condition
- Chronic lung disease
- Cystic fibrosis
- Diabetes Mellitus
- Gastrointestinal Disorders
- Hearing Disorders
- Neurological and neurosurgical disorders including epilepsy, hydrocephalus
- Orthopedic and/or neuromuscular disorders including cerebral palsy, scoliosis
- Visual disorders including cataracts, glaucoma, and strabismus

Cordele office: Crisp, Dooly, Macon, Marion, Schley, Sumter, Taylor, and Webster
