CHILDREN READY TO ENROLL IN SCHOOL OR DAYCARE

 The Columbus Health Department urges parents not to wait until the registration rush to get their children vaccinated for the new school year.  Muscogee County Schools begin on August 8, 2011.  Parents are encouraged to avoid the rush and come in now  to the health department for Vaccinations, Eye, Ear and Dental Certificates.

Georgia law requires children enrolling in day care or school to have their required vaccinations.  A child entering a day care center or private or public school in Muscogee County, for the first time, must have two key forms: Georgia immunization form 3231(which must be completed by a health department or doctor) and Georgia form 3300, documenting vision, hearing and dental screenings. Parents are reminded to bring their child's immunization record to the health department as well.

 Also needed for your child to start school is a certified birth certificate.  Birth certificates of children born in Muscogee County are available from the Columbus Health Department’s Office of Vital Records, 2nd Floor, 2100 Comer Avenue, Monday through Friday between 8:00 a.m. and 4:30 p.m.   The cost for a birth certificate is $25 for first copy and $5 for each additional copy received at the same time for the same child.  A Georgia driver's license or state identification card is required, and either a parent or grandparent, or legal guardian may pick up certificates.
