Children's Services:

Immunization - on going service for early childhood disease: DPT, polio, MMR, chickenpox, Meningitis, and Hepatitis B.

Health check - identify through earlyscreening of children, birth to 21 years of age, handicapping conditions and health problems that could lead to chronic illness or disability. Screening for lead, vision, hearing and dental are vital for this clinic with special emphasis on follow up.

Genetics - provide early detection of birth defects throughblood screening; counseling for families, and referrals for early treatment.

WIC - provide comprehensive health and nutrition services to encourage wellness, healthy lifestyles and preventing health care. Nutrition information services and nutritious supplement foods are avialabel to eligible women, infants and children up to 5 years of age.

Children's Dental Health - cleaning, xrays, sealants, extractions and fillings for school age children. Fee base on income. Medicaid accepted.
Children First - provide a system of identification, linkage, monitoring, access to primary health care and service coordination for children 0-4 who are at risk for poor health and/or development.

"Babies Can't Wait" - provide health education and social services for families of children 0-3 who may be developmentally delayed.

Children's Medical Services - provide treatment and services to children, age 0-21, with special health care needs helping them to attain/maintain their optimum level of health and functioning.

Adult Services:

Immunization - an ongoing service to prevent communicable diseases and promote health for the general public - includes flu and pneumonia shots. ( seasonal)

STD Screening and Treatment - diagnose and treat patients with STDs, confidential screening and treament of SDTs, counseling and partner notification for HIV/AIDS.

Senior Referral Service - telephone reassurance five days a week to those homebound or lonely, health screening, and locate speakers on aging topics.

Hypertension/Diabetes Services - offer detection, treatment, monitoring, referral, follow-up and patient education to some of the chronic disease such as hypertension, diabetes, hyperlipidemia, colon, rectal, cervical, pelvic and breast cancers.

Pregnancy Related Services:

Postpartium care - provide home visits and counseling for the postpartium mom, nutrition education for mom and newborn, breastfeeding techniques and basic baby care.

Presumptive Eligibility - gives same day Medicaid to clients and affords them the opportunity to seek early prenatal care.

Perinatal Case Management - provide client centered goal oriented process to assessing the total needs/services for pregnanat women obtaining these services and follow-up through monthly contact and monitoring, referrals, and more.

District Services:

Teen Clinic - Promotes optimal health in all teens, males and females up to 19 years of year; provides abstinence counseling and various birth control methods to prevent or delay teen pregnancy; nutritional counseling; and referrals as needed. All visits are CONFIDENTIAL.

Primary Care - Nurse Practitioner provides acute, chronic, and preventive care services to patients. Fees based on income.

Health Education - Provides health education to interested persons, organizations, and agencies on various health topics. We can provide pamphlets, resource speakers, assistance with program planning, information, and a referral source.

District Clinical Services - Reduces the spread of HIV/AIDS, STDs amd TB through education, counseling, testing, partner notification, surveillance, case managment, and clinical services.
